Are you thinking of MUSHA maybe?
This was one I actually only played at a friends at one point or two myself. Can't remember if my friend owned it or it was a rental, but we played it all night trying to conquer it.

Nah it wasn't MUSHA unfortunately. This one scrolled horizontally. I'd say it was Darius II/Sagaia, since that's the only one I can think of with a biomechanical fish as the first boss. However that doesn't add up because Darius II doesn't have a "kill everything" bomb or weapons with configurable firing angles.

Forgotten Worlds lets you configure your angles any which way, and level 2 has a water theme. The boss is a kind of sea-dragon, which you might remember as a fish? It's also 2-player.

When I was maybe 6 years old I went to a kid’s house and he had this shmup on the Sega Genesis. I mostly remember you could swap the shooting angle of your bullets (straight ahead, vertical, other weird angles). Also the boss on stage one was a fish. There was a “kill everything” power up we’d save for the fish cause we couldn’t kill him otherwise. Any idea what this game might have been? The fish boss definitely sounds like something you'd see in Sagaia. The shooting mechanics sound similar to Bio-Hazard Battle.

The only other thing I can remember is that the first stage was in space. I pretty distinctly remember inky blackness with stars. Then a few minutes into the stage your ship went inside an enemy space station or perhaps a hollowed out asteroid fortified with enemy units or something. I can't remember much else. We were really bad at it, so we could never progress any farther than the first stage.

I don't know about the fish boss, but it sure does sound like it might be Hellfire. You've got the multi-directional fire thing going there, it starts in space and transitions to a station.
