I usually use headphones when gaming. For portables, I use them just about all the time. For consoles, if I have the house to myself I'll use the speakers and subwoofer I have hooked up specifically for gaming, same of course goes for if someone is actually gaming with me. On average, though, my wife is watching tv or youtube about 10 feet away from me, and its hard to get immersed when hearing that stuff while gaming, so I'll go headphones.
Also feels like the range in quality is higher with headphones. Even fairly mediocre headphones provide a pretty good sound quality compared to speakers. Streets of Rage 3 gave my old speakers hell, and even dollar store earbuds carried the sound better. Only downside for me personally, is that I tend to get achy when wearing my preferred headphones for too long, since they are quite heavy.
I think my ideal scenario would be speakers, there is something I really like about it, gives me arcade-y vibes just having the visuals and audio on full display.Its just not an ideal setup most of the time, and headphones are better than sound waves from multiple sources clashing midair.

Pretty much always these days. Having to listen to a game's repetitive sounds and music when you're not the one playing annoys me so I wouldn't want to inflict it on anyone else. It also tends to be more immersive anyway.

The option closest for me is never using headphones. In truth, I will occasionally use them when playing handheld. I never, ever use them on consoles, but I have a pretty decent soundbar/subwoofer. If I were using terrible speakers, I'd definitely swap to headphones.

I bought a nice pair of Sennheiser HD 579's, back in October last year to finally replace my ancient 10 year old 555's that were falling apart. But I honestly haven't used them much because it finally hit me, I think they give me headaches briefly after some use. I sucked it up while playing RE2 Remake to really get in some immersion, so ideally I've always liked headphones a lot for horror games and whatnot. But yeah... comfort has become an issue over the years. I have two pairs of earbuds I tried out a few months back too but I'm still not sure what to think.
I used headphones a lot when I lived at home in my later teen years. But it's been very sparse and random in my adult life. Ideally I'd like a good soundbar or sound system in general someday, but it seems pointless to invest much in that while living the apartment life since I try to be a respectful neighbor.
The bluetooth wireless ones always look nice, but are expensive and I've been too lazy to try some out. Plus with my recent experiences, it seems like it's becoming harder and harder to find any "comfort" with headphones in general.

I try to use earbuds whenever possible. Mainly because it blocks out other noises. Helps with the immersion. Unfortunately one of my TVs (the one I use for 7th gen gaming) doesn't have a headphone output. But I'll generally use earbuds for everything else. Also portables tend to have crappy speakers, so earbuds matter for actual fidelity there. Edit: I don't like headphones in this style...
...for multiple reasons. First they press hard against my glasses' legs, smushing them into my face annoyingly. Second the encapsulating cushions act life ear muffs, making my ears get uncomfortably hot. Lastly the headband part pushes down on my head like a helmet and irritates my hair. I just don't like this style at all. Earbuds are far less intrusive and comfortable for me.

I bought this for those HD 579's and it worked perfectly, assuming your position to the TV / cable length isn't an issue or whatever:
My Samsung's have always had some kind of audio out, but you could never control the volume levels... so that was really annoying. Things were either whisper quiet or painfully loud.

Looks more versatile than what I got.
I just played some ME3 tonight with headphones, should have been doing this from the start. Sci-fi awesomeness always goes well with good sound too.

I almost always use earbuds for handhelds, but I never use any kind of headphones for gaming otherwise. A decent set of earbuds is gonna sound a zillion times better than the crappy little speaker in the handheld. There's also stereo versus mono. Even handhelds that have stereo speakers, there isn't enough separation. In essence, I only use headphones for handhelds so I can get better quality. I don't have that problem with consoles, so I don't use headphones.
