Haha, yeah I don't think I beat the final stage/boss without save states when I ran through some Natsume games a few years ago.

He's surprisingly easy, I actually got him in one go. I feel like they were really channeling their inner Batman NES with that vertical climb at the end. Well-timed super suit and all that.

Yeah the Natsume NES games are stellar, most are at least. Shatterhand is certainly a game worth re-beating. I did re-beat it myself not long ago:

Ex Extermination was made by Deep Space, the same guys that made that Japanese-exclusive game Hungry Ghosts that you sometimes bring up. The company started by the ex- '80s Capcom game designer that made Tomba. Swery was one of the game designers. It doesn't seem that original in comparison, but from what I tried the atmosphere was there. After playing it, I'd say I agree with your take. It's a pretty standard survival horror game from that era with the atmosphere as a highlight. It did some pretty neat things with it, but nothing earth shattering, more like nice touches. The same goes for the story, it's straightforward with one or two twists to keep it from getting too stale. The combat is serviceable, I could mention some nitpicks (like the auto-aim being useless for aiming at weak spots) but still perfectly playable. The more interesting part is the localization as it received two separate ones for the American and European market. The character model for the protagonist is completely different and they both come with their own dub, with a small sampler here:
The PAL version's voice acting is strange, sometimes they stretch the voice track to match the allotted time and feels very stiff at times, but at the end I think I liked it. It felt like it fit in the overall feeling the game evoked. Also from looking at some videos it seems that in the American version the monsters have less HP (which might be a first?) but the game balances that with having smaller magazines upgrades for your gun (30 bullets per upgrade instead of 50) and having less health packs laying around. In the end, with its atmosphere and short length, it's worth a playthrough.
